Medication Management
Expiration dates (sample meds, clinic meds, fridge meds, E-kit meds). Check expiration dates within the first 2 weeks of the month and REMOVE any medications that are expiring that month.
Multidose vials - expire 28 days after first use. Must label with initials, date opened, and expiry date.
EXCEPTION TO THIS IS POLIO VACCINE - use manufacturer's expiration date.
Must also remove expired medications from AccuShelf kiosk(s)
Ordering.
Medications and private vaccines are currently ordered by Nursing leadership. Connect with your nursing supervisor or manager to learn the process for your clinic.
May only order medications on formulary.
Non-pharmacy items include IUD's, Nexplanon, phenol swabs, and Drysol (aluminum chloride). Request this order from site's Coupa user (usually site supervisor or lead MA).
Record-keeping
If transferring medications from one clinic to another, create a transfer in AccuShelf to document.
All packing slips should be signed, dated, and kept in the clinic RN binder for at least 3 years.
All injectable medications, including IUD's, should be added to eCW inventory.

Note: the clinics do not carry blood pressure medication. Requests for medications from clinicians should either be directed from the clinician to their medical leader, or from the nursing team to their supervisor.
If a medication has been fully dispensed from a syringe or vial, the remaining residual medication in the needle, syringe, or vial is considered used for its intended purpose and is not a pharmaceutical waste but rather sharps waste
Bins must be picked up when full or 1 year after accumulation start date.
White bins: Met-Bio will pick up

There are three types of pharmaceutical waste:
Pharmaceutical (white bin)
RCRA (black bin)
Narcotic (RX Destroyer)
All pharmaceutical waste should be logged on the appropriate form, then placed in the correct container. Remove as much packaging as possible to minimize bulk. Don't put loose pills or liquids in the container.
Vaccine waste: VFC/VFA/State wasted/expired vaccines should be returned to the appropriate program. *Some* private vaccines may be able to be returned to the manufacturer, but most will need to be disposed of in Pharmaceutical or RCRA waste.
All non-RCRA medications should be disposed of in the white bin.
Disposal Log/Pharmaceutical Waste Log
All medications being disposed of - whether pharmaceutical or RCRA - must be recorded on a log. Complete all fields of the log.
There should be a different log for each bin.
May need additional pages of the log for the bins as disposed medications accumulate.
Start a new log EVERY time a new bin is received.
Keep records for 3 years.
All RCRA medications should be disposed of in the black bin. Examples include:
Warfarin
Nitroglycerin spray/ointment
Nicotine cream or sprays
Insulins that contain cresol
Inhalers
Silver nitrate sticks (unused)
Alcohol content over 24%
Rubbing alcohol
Ammonia inhalant
Ear drops
Heavy metals:
Chromium/selenium (Multivitamins)
Silver (Silvadene cream)
Vaccines (thimerosal)
Engerix
Multi-dose Flu
Tetanus
This is intended for disposal of expired or unused controlled substances (buprenoprhine) prescribed through the Medication Assisted Treatment (MAT) program. Not all clinics will have this. This is NOT for patients to drop off their controlled substances for destruction.
