Inform the patient that you are making the report.
Call the local enforcement agency where the incident occurred to make the report by telephone.
Fax form OES 2-920 to the law enforcement agency within two working days.

Both DS-326 and CDPH 110c are California forms used to report medical conditions that may affect a person’s ability to drive—but they serve different purposes and are used by different agencies.
DS-326 is a comprehensive evaluation tool used when the DMV is actively reviewing a driver’s medical status and should be completed by the person's PCP.
CDPH 110c is a mandatory reporting form for certain conditions that may trigger DMV involvement and may be submitted by RN/LVNs.

If the patient does not need emergent care, do not send them to the emergency department. It is more time with potential loss of evidence.
Determine where the sexual assault (SA) occurred to know which LE agency to call.
Determine when the SA occurred.
It is not our job to decide if the patient is within the “window” for an exam. That is up to law enforcement and the SAFE RN.
If it is outside of the 5 day window, the patient can still get the assault documente and/or investigated by law enforcement.
